If you cannot use the internet
You can:
- Call us on 01462 414839 and complete the form over the phone.
- Ask for a paper triage form at reception.
What happens next?
- The form is available Monday to Friday, 8:00am to 6:30pm.
- A doctor will review your form.
- We will contact you the same day.
- The form stays open all day and does not close when a certain number of requests have been received.
How to order a repeat prescription
When you completed your new patient questionnaire, you told us whether you take regular medication.
If you take repeat medication, our Pharmacy Technician will add these medicines to your record. This can take up to 2 weeks after we receive your questionnaire.
You can order your repeat prescription in the following ways:
- Online using our administration form https://accurx.nhs.uk/patient-initiated/E82008
- Through our website Visit our Prescriptions page on the Nevells Road Surgery website.
- Through the NHS App Before you can use the NHS App for prescriptions, you must complete our Register for Online Services form: Register for Online Services – Nevells Road Surgery
- Using a paper form Complete a repeat prescription form and hand it in at reception.
