Can my child have an ADHD assessment via the NHS?
Yes, however the current waiting time is around 3 years. If you wish to have an NHS referral for a Child ADHD assessment, please complete all three following forms:
ADHD Autism CYP Referral Herts Part 1 of 3 Parent Carer Questionnaire v1.0 Sept25 FILLABLE
ADHD Autism CYP Referral Herts Part 2 of 3 Parent Carer Questionnaire v1.0 Sept25 FILLABLE
ADHD Autism CYP Referral Herts Part 3 of 3 School Education Setting Questionnaire v1.0 Sept25 F
Once completed you can return the completed forms to the reception desk at Nevells Road Surgery marked FAO secretaries or send to our surgery email address [email protected]
What happens if I start an NHS referral for a child ADHD assessment when they are under the age of 16, but when they get to the front of the waiting list, they are now over the age of 16.
This will be reviewed by the NHS Child Development team themselves. There is the possibility that because the patient is over the age of 16, this could lead to the referral being cancelled. This is outside of Nevells Road’s control.
